Sexual Abuse Survivor Proofs of Claim may be submitted electronically on this website or by mail so that they are actually received by Omni Agent Solutions on or before the Sexual Abuse Claims Bar Date of November 16, 2020 at 5:00 PM (EST), which is the deadline established by the Bankruptcy Court.

If you have a claim arising from other types of abuse, including non-sexual physical abuse, non-sexual emotional abuse, bullying or hazing, OR if you have a claim arising from sexual abuse and you were at least eighteen (18) years of age at the time the sexual abuse began, you should consult the Notice of Deadlines Requiring Filing of Proof of Claim and file a General Proof of Claim (Official Bankruptcy Form 410).

If you have any questions about how to fill out your Sexual Abuse Survivor Proof of Claim, you should review the instructions at the beginning of the claim form. You can also call the restructuring hotline at 866-907-BSA1 (2721). Otherwise, you will need to consult with your own legal advisor.

Boy Scouts of America Sexual Abuse Claim Submission

Omni Agent Solutions has created this claims page for the secure submission of Sexual Abuse Claims in the chapter 11 cases of the Boy Scouts of America and Delaware BSA, LLC, as set forth in the Order, Pursuant to 11 U.S.C. § 502(b)(9), Bankruptcy Rules 2002 and 3003(c)(3), and Local Rules 2002-1(e), 3001-1, and 3003-1, (I) Establishing Deadlines for Filing Proofs of Claim, (II) Establishing the Form and Manner of Notice Thereof, (III) Approving Procedures for Providing Notice of Bar Date and Other Important Information to Abuse Survivors, and (IV) Approving Confidentiality Procedures for Abuse Survivors (the “Bar Date Order”). Pursuant to the Bar Date Order, all information submitted will be treated as personal confidential information, maintained securely and will be unavailable to the public, unless you choose on your Sexual Abuse Survivor Proof of Claim to release this information.

Upon the successful submission of your claim, you will receive a confirmation email that your claim was successfully submitted to the email address you provide above. Please provide a claim identification code that will appear in the confirmation email you receive. This will be the only way to identify your claim until you receive a claim number from Omni Agent Solutions within the next 7 calendar days. The Claim Identifier must only contain numbers, letters and hyphens, up to a total of thirty (30) characters. PLEASE DO NOT USE PERSONAL INFORMATION SUCH AS YOUR NAME OR SOCIAL SECURITY NUMBER.

Supporting documentation is limited to a single PDF attachment that is less than 5 megabytes in size and under 100 pages.

If you submit your claim online, you do not need to submit a claim by mail.

Proofs of claim may be submitted in person, by courier service, hand delivery or mailed to Omni Agent Solutions at the above address.

Submission of proofs of claim forms by email and/or facsimile transmission is not permitted.

You must submit an originally executed proof of claim.

If you would like a copy of your claim returned to you as proof of receipt, please enclose an additional copy and a self-addressed postage-paid envelope.

Privacy of Information Collected

In this case, the Bankruptcy Court has set up a procedure to protect your privacy. In order to protect your privacy, you must file according to the directions above. Sexual Abuse Survivor Proofs of Claim will not be available to the public unless you choose to release that information. Unless you elect otherwise on the Sexual Abuse Survivor Proof of Claim, your identity and your Sexual Abuse Survivor Proof of Claim will be kept confidential and outside the public record, and will only be confidentially provided to the Permitted Parties as defined in the Bar Date Order pursuant to Bankruptcy Court-approved guidelines.

The information requested on the proof of claim form is being collected for the purposes of facilitating the debtors’ bankruptcy cases under the U.S. Bankruptcy Code and processing any claim you may have against the debtors.
