Sexual Abuse Survivor Proofs of Claim may be submitted electronically on this website or by mail so that they are actually received by Omni Agent Solutions on or before the Sexual Abuse Claims Bar Date of November 16, 2020 at 5:00 PM (EST), which is the deadline established by the Bankruptcy Court.
If you have a claim arising from other types of abuse, including non-sexual physical abuse, non-sexual emotional abuse, bullying or hazing, OR if you have a claim arising from sexual abuse and you were at least eighteen (18) years of age at the time the sexual abuse began, you should consult the Notice of Deadlines Requiring Filing of Proof of Claim and file a General Proof of Claim (Official Bankruptcy Form 410).

Submission of proofs of claim forms by email and/or facsimile transmission is not permitted.
You must submit an originally executed proof of claim.
If you would like a copy of your claim returned to you as proof of receipt, please enclose an additional copy and a self-addressed postage-paid envelope.
Privacy of Information Collected
In this case, the Bankruptcy Court has set up a procedure to protect your privacy. In order to protect your privacy, you must file according to the directions above. Sexual Abuse Survivor Proofs of Claim will not be available to the public unless you choose to release that information. Unless you elect otherwise on the Sexual Abuse Survivor Proof of Claim, your identity and your Sexual Abuse Survivor Proof of Claim will be kept confidential and outside the public record, and will only be confidentially provided to the Permitted Parties as defined in the Bar Date Order pursuant to Bankruptcy Court-approved guidelines.
The information requested on the proof of claim form is being collected for the purposes of facilitating the debtors’ bankruptcy cases under the U.S. Bankruptcy Code and processing any claim you may have against the debtors.
